Much of season 10 has focused on Dean Winchester dealing with the Mark of Cain. He was a demon for the first three episodes of the season because of it, and even though he has since been cured , he’s still feeling the effects.
In the mid season final, Dean, compelled by the Mark of Cain, brutally killed off several unsavory, although non-supernatural, humans. Now, when the mid season premiere picks up, Dean will have to deal with the fallout of his actions. The clips released for the episode show Dean is willing to do whatever it takes to get the mark off his arm.
Unfortunately, the only text that might hold a cure is the Demon Tablet and not only is it missing, but there are also no prophets left who can decipher it. The only other option is to seek advice from the man who wrote It, Metatron (Curtis Armstrong), and he just happens to be returning to the series.

THE AFTERMATH — After the massacre at the ranch, Dean (Jensen Ackles), Sam (Jared Padalecki) and Castiel (Misha Collins) double their efforts to find a cure for the Mark of Cain but there is nothing in the lore that can help them. Desperate, Castiel comes up with an idea that Sam deems the “single worst idea I’ve ever heard.” Meanwhile, Rowena (guest star Ruth Connell) continues to plot against Crowley (Mark Sheppard), and Castiel tries to mend his relationship with Claire (guest star Kathryn Love Newton) who is plotting revenge against Dean for killing her friend.
